COSMETIC TREATMENTS
I understand that I will receive spa or cosmetic medical treatments from Skin Blends. I understand that depending on the treatment I select, I will be required to sign an informed consent specific to that treatment. I am fully aware that my condition is solely of a cosmetic nature and that the decision to proceed is based on my expressed desire to do so. I will not hold Skin Blends LLC, its owners or its employees responsible for the results I experience.

CANCELLATION AND NO SHOW POLICY
When your appointment is scheduled, we set aside that time specifically for you. Because your services are guaranteed reservations, we require a credit card to hold those times reserved for you. We understand that sometimes it is necessary to re-schedule an appointment. In the event that you are unable to give 24 business hours of notice, a cancellation fee of 100% of the scheduled procedure(s) will be charged to your credit card.  All prepaid packages and/or Groupon appointments will result in the forfeit of one treatment from your package. We regret any inconvenience this may cause. 

LATE POLICY
If you arrive after your appointment was scheduled to begin, you will be seen but only for the amount of time remaining. If you are 20 minutes late for a 20-minute procedure, this will be considered a missed appointment. We regret any inconvenience this may cause. Please plan to come in early and have time to try our skin care products and browse our showroom.

CONSENT TO PHOTOGRAPH
I consent to be photographed during the course of my treatment with Skin Blends LLC. The photographs are to track the progress of my treatments and are part of my treatment records and therefore are the property of Skin Blends LLC. I consent to the use of my photographs for marketing, research, educational and/or scientific purposes. I understand that every attempt will be made to protect my identity and my name will not be disclosed.

GROUPON POLICY
Groupons are for FIRST TIME CLIENTS only! We deliver outstanding service and have great prices

PET POLICY
Per state Board regulation, only service animals are allowed at our establishment.

CHILDREN POLICY
For safety reasons and insurance purposes, no child under the age of 18 may have a service without written parental consent. Also, no child under the age of 12 may accompany you into the treatment room while you are having a service. If a child under the age of 12 is on premises during the time of your treatment, they must be accompanied by an adult for supervision and remain in the showroom. We do not have the facilities to care for children. We love children and hope you understand that we cannot assure their safety in a professional environment.

RETURN AND REFUND POLICY
All sales of skin care and makeup products are final. Unopened products may be returned with a receipt for a credit within 30 days. Pre-paid packages are non-refundable and non-transferable. 

TIPPING POLICY
Our employees make their living by providing excellent services deserving of appropriate tips. If your service is not satisfactory, please let us know. Otherwise, please tip appropriately.
